摘　　要：目的:比较纤维支气管镜(以下简称纤支镜)检查时用丙泊酚靶控输注与手控输注麻醉的区别。方法:行纤支镜检查病人50例,随机分为两组。全部病人诱导剂量丙泊酚1·5~2·0mg/kg静脉推注后,丙泊酚靶控输注(PTCI,T组)连接TCI-I型微泵,以丙泊酚30~75μg·kg-1·min-1维持,丙泊酚手控输注(PMCI,M组)必要时分次追加丙泊酚1·5~2·0mg/kg。观察诱导后、插管中(入声门)、镜检中、镜检结束及完全清醒的平均动脉压、心率、血氧饱和度。记录给药的时间及完全清醒的时间。结果:T组比M组药物作用稳定,可控性强,纤支镜插管顺利(23例对17例),插管失败率低。T组平均动脉压和心率等波动明显比M组缓和(P<0·05)。结论:丙泊酚联合咪哒唑仑、芬太尼用于纤支镜检查,PTCI比PMCI血液动力学稳定,是一种比较安全的麻醉方法。Objective: To evaluate target-controlled infusion and manual-controlled infusion of propofol combined with midazolam and fentanyl for the general anesthesia of patients under fibrobronchoscopy. Methods: Fifty patients, who were ASA Ⅰ-Ⅲ, were randomly divided into 2 groups, each group contained 25 cases. All of the patients were induced with infusing propofol 1.5-2.0 mg/kg. In propofol target-controlled infusion group (PTCI, group T), the patients were kept infusing propofol 30-75 μg·kg^(-1)·min^(-1 ) with TCI-I model micro-pump. In propofol manual-controlled infusion group (PMCI, group M), the patients received infusing propofol 1.5-2.0 mg/kg each time if necessary. The mean arterial pressure (MAP), heart rate (HR) and oxygen saturation (SP(O2)) were observed after induction, during the intubation, after examination and complete lucidness. The time of giving medicine and complete lucidness were recorded. Results: The effect of the drug in group T was more stable and controllable than that in group M. The fluctuation of MAP and HR of group T was significantly milder than those of group M (P<0.05). Conclusion:[WTBZ] Porpofol combined with midazolam and fentanyl could be used for general anesthesia of patients under fibrobronchoscopy. The hemodynamics of PTCI is more stable than that of PMCI. The PTCI is a kind of safe general anesthesia.
