摘  要:目的:观察丙泊酚、咪达唑仑在心肌缺血再灌注损伤时c jun基因表达的影响,从基因表达水平探讨它们对心肌保护作用的机制。方法:健康SD大鼠36只,每组12只。在心肌缺血15min后分别给生理盐水(A组)、丙泊酚(B组)、咪达唑仑(C组)再灌注,时相为Ⅰ0min、Ⅱ15min、Ⅲ30min、Ⅳ60min、Ⅴ120min、Ⅵ240min,各时相取2只大鼠,多聚甲醛灌注固定后取缺血心肌标本作石蜡切片,免疫组化方法检测c jun基因表达,显微镜下计数阳性细胞,拍片。结果:各组Ⅲ、Ⅳ、Ⅴ、Ⅵ时相与Ⅰ时相相比c jun基因表达明显增强(P<0.05);A组Ⅲ、Ⅳ、Ⅴ时相c jun基因表达均比B、C组增强(P<0.05),统计学有显著性意义。结论:丙泊酚、咪达唑仑对缺血再灌注损伤有一定的保护作用,为心肌缺血再灌注损伤时安全可用的麻醉药,其作用与缺血再灌注时间有关,即在30~120min内对缺血再灌注心肌有明显保护作用,这对临床选择合理时间窗用药有一定的指导意义。Objective: To observe the effects of propofol and midazolam on the expression of c-jun gene during myocardial ischemia and reperfusion injury, and try to find their relationship of protective myocardial ischemia in gene lever. Methods: Thirty-six adult SD rats, the left anterior coronary descending branch was ligated in 15 minutes, then the ligation was removed, were divided into 3 groups: group A control(NaCl); group B (propofol); group C (midazolam). All groups divided 6 time phases toget myocardial tissue at Ⅰ0 min、Ⅱ15 min、Ⅲ30 min、Ⅳ60 min、Ⅴ120 min、Ⅵ240 min. Immunohistochemistry technique was used to analyze the amount change of the c-jun gene express. Results: c-jun gene expression postive cell calculation at each other time phase was increased than Ⅰtime phase.Amount of phase Ⅲ、Ⅳ was significant higher thanⅠ(P<0.05);Group A c-jun gene express were significant higher than group B and group C in timeⅢ、Ⅳ、Ⅴ(P<0.05). Conclusion: Propofol and midazolam decreased myocardical ischemia reperfusion injury c-jun gene expression and protected myocardical function. This protect function was correspond with reperfusion time, which was important for chance to use drugs in clinic work.
