放射性核素心血池显像测定冠心病患者左室功能的价值

摘　　要：目的:探讨放射性核素心血池显像测定冠心病(CHD)患者左室功能的临床价值。方法:使用Elscint公司SP—6型SPECT,采用体内法标记红细胞平衡法测定,应用Elscint公司心功能处理程序对数据处理,产生如下的心功能参数:LVEF(左室射血分数)、PER(高峰射血率)、PFR(高峰充盈率)。结果:对照组各项指标与文献报道结果相近,AP组PER、PFR与对照组比较差异有显著性(P<0.05);LVEF与对照组比较差异无显著性(P>0.05)。MI组LVEF、PER、PFR与对照组AP组比较差异均有显著性(P<0.01);CHD患者中有18例(37.5%)EF值下降,而PFR为28例(58.33%),后者检出率高于前者,差异有显著性(P<0.05),LVEF正常患者,PFR降低者12例(40%)。结论:PFR对于反映CHD患者左心功能损害较LVEF更灵敏,且可反映患者心肌缺血的严重程度,对于临床上疑为CHD的患者,如果左室收缩功能正常,而舒张功能降低,有助于CHD的诊断。Objective:To investigate the clinical value of radio nuclude cardiac blood-pool imaging in detecting left ventniculr function coronary heart disease (CHD).Methods:SPECT type SP—6(made by Elsint Company) was used and the detection made by internal labelled erythrolyte equilibrium method,and the data processed using cardiac function processing programme by Elscint Company and the following parameters obtained. Left ventricular ejection fraction(LVEF),peak ejection rate(PEK),and peak filling rate(PFR).Results:The indexes of the control group wene similar to those reported by documents. There was a significant difference in PER and PFR between AP Group and the control group( P< 0.05);no significant difference was observed in LVEF between them( P< 0.05).There was significant difference in LVEF,PER,and PFR between MI Group and the control group and AP Group( P< 0.01). Decreased EF value was found in 18 cases(37.5%) and PFR in 28 cases(58.33%) in CHD patients,the latter was significantly higher than the former. There was significant difference( P< 0.05). Of patients with normal LVEF,decreased PFR was found in 12(40%). Conclusion:PFR is more sensitive than LVEF in reflecting the damage of left ventricular function,and it may also reflect the degree of myocardial ischemia. For suspeted CHD,normal contraction function of left ventricular and decreased diastolization function are helpful to the diagnosis.
