高频彩超诊断隐睾的价值

摘　　要：目的:探讨高频彩超诊断隐睾的价值。方法:对18例患侧阴囊空虚无睾丸的患者采用高频彩超沿着睾丸下降走行的途径探查,重点观察阴囊上方腹股沟区、盆、腹腔。其中17例经手术证实。结果:18例隐睾患者中,16例在腹股沟部;1例于右侧腹腔探及低回声结节,周围有网状组织包绕,术后证实是大网膜包裹;1例于腹膜后探及低回声结节。结论:隐睾手术证实确诊率94%,89%的隐睾位于腹股沟部,且隐睾位置表浅,受肠气影响小,随腹压的增加可见其蠕动,所以高频彩超是诊断本病的最好手段,可提供睾丸的位置、大小及形态,对明确诊断、鉴别其有无恶变、尽早决定治疗方案、使其降入阴囊及促进其发育等有极为重要的价值。Objective: To observe the value of high-frequency color ultrasound Doppler in the diagnosis of cryptorchidism. Methods: We used ACOSON ASPEN color ultrasound Doppler and the frequency of probe was 5～10MHz.The patients having no testis in the affected scrotum were examined along the testis descending way, especially groin,pelvic cavity and abdominal cavity. The solid echo rolls similar to the shape of testis were found and they squirmed along with the change of abdomen pressure. 18 cases of cryptorchidism were analyzed. 17 cases in the test group were confirmed by operation. Results: Of 18 cases of retained testis,16 were located in groin; a low echo node was found in right abdomibal cavity of 1 case, around which having reticular tissue which confirmed by operation was greater omentum; a low echo node doubted to be the testis was found behind the peritoreum of 1 case and we (suggested) the patient should take further examination. Conclusion: It is hard to make clear the concrete place of retained testes by clinical examination. Of other examinations the cost is high and the operation is complicated. This test found that there were 89% retained testes locating in groin and the diagnosis rate was 94%. For the places of retained testes are surface and they squirmed along with the change of abdomen pressure, high-frequency color ultrasound Doppler is the best method diagnosing this disease which can provide the place, size and shape of retained testes and it has great value in the respect of having a clear diagnose and deciding therapeutic plan early which can make the retained testes descend into the scrota and promote their growth.
