摘  要:目的 探讨兔严重烫伤 (5 0 %Ⅲ度 )早期血脑屏障开放的最早时间、部位及特征。方法 雄性大白兔 4 2只 ,随机分成 6组 (对照组、烫伤后 1、2、2 .5、3、4小时组 ) ,烫伤组均制成 5 0 %Ⅲ度烫伤模型。采用12 5I标记的牛血清白蛋白 (12 5I BSA)作为示踪剂 ,并与各组电镜结果进行对比观察。结果 12 5I BSA的在体实验证实 ,烫伤后 2 .5小时双侧前叶、小脑的比放射性较对照组有显著性增加。电镜结果表明烫伤后 2小时 ,双侧小脑、前叶皮质部分毛细血管周围出现水肿。结论 兔 5 0 %Ⅲ度烫伤后 2小时 ,双侧前叶和小脑处血脑屏障出现开放 ;烫伤后 3~ 4小时 ,脑组织出现弥漫性血脑屏障开放。12 5I BSA测定微血管通透性灵敏度高 ,可精确地定量分析 ,全面、确切地反映早期血脑屏障的病理变化。Objective To explore the earliest time,position and characteristics of the opening BBB in the early stages of severely scalded rabbits.Methods Forty-two rabbits were randomized into control、1、2、2.5、3 and 4h groups after scald injury.The rabbits in wounded groups were inflicted with 50% TBSA of Ⅲ degree skin scald. 125 I labeled bovine serum albumin ( 125 I-BSA) was used as tracer to observe the earliest time point and position of opening of BBB after severe scald.The results were compared with the changes of ultrastructure by electronic microscopic.Results 125 I-BSA intake began to increase remarkably at 2.5 hour in anterior cortex and cerebellum,and it became increase remarkably at 3 hour in all parts of the brain.Histological changes of BBB were observed as early as 2 hour after scald,accompanied by swelling of basal lamina and glial cell.The tight junction of blood vessel endothelial cell began to loose and became vague at 3 hour after scald.Conclusion The earliest opening of BBB is at 2 hour after scald,and the position is at cerebellum and anterior cortex.These facts suggest that the impairment of BBB would aggravate in a diffuse and progressive way. 125 I-BSA is sensitive and precise to monitor the permeability of BBB.
