摘  要:目的:观察血管内皮细胞生长因子对大鼠任意皮瓣断蒂时间的影响。方法:实验于2003-04/2004-04在华北煤炭医学院中心实验室完成。采用SD大鼠背部3cm×7.5cm单蒂任意皮瓣为模型,皮下局部注射内皮细胞生长因子后,通过计算机图像分析技术对不同断蒂时间皮瓣成活率的测定、微血管密度的测定、组织学检查、碱性成纤维细胞生长因子免疫组织化学染色等手段来观察内皮细胞生长因子对皮瓣断蒂时间的影响并探讨其机制。结果:局部应用内皮细胞生长因子后,皮瓣断蒂时间由7d缩短为5d;组织学检查发现内皮细胞生长因子能促进皮瓣毛细血管新生;生理盐水组微血管密度中1/3段为(26.83±1.96)个/cm2,远1/3段为(26.96±2.11)个/cm2,内皮细胞生长因子组中1/3段为(30.99±1.93)个/cm2,远1/3段为(36.11±2.32)个/cm2,表明内皮细胞生长因子能增加皮瓣中远段微血管密度。生理盐水组碱性成纤维细胞生长因子阳性细胞数中1/3段为(22.2±3.5)个,远1/3段为(18.4±4.5)个,内皮细胞生长因子组中1/3段为(30.8±3.2)个,远1/3段为(33.2±5.1)个,表明内皮细胞生长因子能增加皮瓣中远段碱性成纤维细胞生长因子的表达。结论:内皮细胞生长因子能促进大鼠任意皮瓣早期断蒂,其机制可能与内皮细胞生长因子直接促进血管再生及通过增强另一血管生成因子碱性成纤维细胞生长因子的表达间接促进血管新生有关。AIM:To investigate the effect of vascular endothelial growth factor(VEGF) on pedicle division time in the rat random pattern skin flap. METHODS:The experiment was conducted in the Central Laboratory of North China Coal Medical College from April 2004 to April 2005.A caudal based random skin flap of 3 cm×7.5 cm was raised on the back of SD rat.After subcutaneous injection of VEGF,detection of survival rate of flap at different division time,measurement of microvessel density(MVD),histological examination,immunohistochemical staining of basic fibroblast growth factor(bFGF) were performed by using computerized image analysis technology to observe the influence of VEGF on the flap division time and probe into its mechanism. RESULTS:The time of pedicle division was shortened to 5 days from 7 days after local injection of VEGF.As shown by histological examination,abundant capillaries were noticed by application of VEGF.The MVD in the normal saline group was(26.83±1.96)per cm2 in the 1/3 middle segment and (26.96±2.11)per cm2 in the 1/3 distal segment;That in the VEGF group was(30.99±1.93)per cm2 in the 1/3 middle segment and (36.11±2.32) per cm2 in the 1/3 distal segment,indicating that VEGF increased the MVD in middle and distal segments of flaps.The number of bFGF positive cells was 22.2±3.5 in the 1/3 middle segment and 18.4±4.5 in the 1/3 distal segment in the normal saline group;That in the VEGF group was 30.8±3.2 in the 1/3 middle segment and 33.2±5.1 in the 1/3 distal segment,showing that VEGF could upregulate the expression of bGFG in the middle and distal segments of flaps. CONCLUSION:VEGF can improve pedicle division of rat random pattern skin flap at an early stage.The mechanism might be that VEGF can accelerate angiogenesis directly and upregulate the expression of bFGF in an another vessel to augment revascularization indirectly.
