摘 要:目的:腰椎融合术结合腰椎间融合器植入对治疗腰椎退行性疾病具有维持椎间隙高度、减轻神经根压力、维持脊柱稳定度等优势。介绍各种不同椎间融合器在生物力学、材料及外形上的特点,以及相关手术内容应符合脊柱功能的生物学要求。资料来源:应用计算机检索NCBIEntrezPubMed1990-01/2004-05相关腰椎间融合器的文章,检索词“lumbarinterbodyfusioncage”,限定文章语言种类为英语。计算机检索万方数据库1990-01/2004-05关于腰椎间融合器的文章,限定文章语言种类为中文,检索词“腰椎间融合器,椎间融合器,腰椎融合术”。资料选择:对资料进行初审,纳入标准:①腰椎间融合器的种类、材料及外形的进展。②各种腰椎间融合器的生物力学及其生物学特性。③腰椎间融合器的临床应用。排除标准:重复研究、综述、Meta分析类文章。资料提炼:共收集到74篇相关文献。25篇文献符合纳入标准。排除的49篇为重复或雷同内容。资料综合:25篇文献中,8篇从历史沿革、材料、形状等角度介绍了椎间融合器逐渐适应根、椎间隙、脊柱等压力的进程;9篇详细的以相关生物力学、组织学等角度,以试验方式证实了腰椎间融合器能够恢复腰椎正的常生理状态;8篇涉及腰椎间融合器在使用中的并发症及手术注意事项。结论:腰椎间融合器从材料学、生物力学等角度的进展,使其在材质、外形设计、手术方式等方面能够较好的满足椎间隙、神经根、脊柱等的压力,维持腰椎正常生理功能的生物学要求。在手术中,合理的选择可以在发挥融合器功能的同时避免并发症的发生。OBJECTIVE:The lumbar spinal fusion combined with lumbar interbody fusion cage for regressive diseases of lumbar spine has such advantages as maintaining the height of intervertebral space,easing pressure of nerve root and maintaining the spinal stabilization.In the present study,we aimed to introduce the characters in biomechanics,material,outline forms of different types of cages,and investigate biological needs of related operations meeting the spinal function. DATA SOURCES:The articles related to lumbar interbody fusion cage published between January 1990 and May 2004 were searched for in NCBI Entrez PubMed using computer with the “lumbar interbody fusion cage',and the language was limited to English.Meanwhile,the similar articles published also between January 1990 and May 2004 were searched for in Wangfang Database using computer with the key words “lumbar interbody fusion cage,intervertebral fusion cage,lumbar spinal fusion',and the language was limited to Chinese. STUDY SELECTION:The articles were primarily checked based on the inclusion criteria:①studies on the progress in the types,material and outline forms of the lumbar interbody fusion cage,②studies on the biomechanical and biological characters of the lumbar interbody fusion cages,and ③studies on the clinical application of lumbar interbody fusion cages;And also based on the exclusion criteria:repetitive studies,reviews and Meta analytic articles. DATA EXTRACTION:Totally 74 articles were collected,including 25 accordant with the inclusion criteria and 49 excluded due to their repetition and similarity. DATA SYNTHESIS:Among the 25 articles,8 were about progress of lumbar interbody fusion cages meeting the pressure needs of root,intervertebral space and spinal column in terms of development,material and shapes,9 verified that lumbar spine could be recovered to normal by using lumbar interbody fusion cages experimentally from the aspects of biomechanics and histology,and 8 concerned the complications and the surgical announcements in the usag
