摘 要:目的:比较结核分枝杆菌L型的3种检测方法的诊断意义。方法:对103例耐多药肺结核患者的痰标本分别采用萋-尼(Ziehl-Neelsen)抗酸染色法、聚合酶链反应(PCR)和结核分枝杆菌L型培养法进行检测。通过计算敏感度、特异度和阳性率指标,判断联合检测的准确程度和可靠程度。结果:结核分枝杆菌L型培养与抗酸染色法联合检测准确程度高于与PCR的联合检测,敏感度和特异度同时达到100%;L型培养法重复检测一次Bayes阳性预告率也达96.2%。结论:结核分枝杆菌L型培养与抗酸染色法联合检测准确程度高,重复性好。Objective:To compare their validities of 3 kinds of detecting methods of L-form of Mycobacterium tuberculosis in patients with multi-drug resistant pulmonary tuberculosis for clinical diagnosis.Methods:3 kinds of Ziehl-Neelsen acid fast sputum smear (Ziehl-Neelsen′S), L-form culture and PCR technique were conducted in 103 cases of multi-drug resistant pulmonary tuberculosis.Their sensitivities(true positive ratio),specificity(true negative ratio), and positive rate were calculated from combinations of 3 tests.Results:Its validity of combination L-form culture with Ziehl-Neelsen′S was higher than that of combination with PCR, its sensitivity and specificity was all up to 100%,and more, predictive positive rate of L-form culture for replication detection was up to 96.2% by Bayes′ formula.Conclusion:Effect of combination L-form culture with Ziehl-Neelsen′S was high in its validity and repeatability, which is fit to facilitate early rapid diagnosis.
