摘 要:目的 探讨兔肝脏撞击伤后血液黏弹性变化的特点及其临床意义和对机体的影响,为临床在肝损伤后早期采用降黏措施提供实验研究依据。方法 建立兔肝脏撞击伤的动物模型,分别在伤前、伤后10、30、6 0、12 0分钟及死亡即时等几个时相点各抽血2ml,2 4小时内全部送检并完成测定,并按血液黏弹性计算方法得出结果。结果 肝撞击伤后当时间一定,血液黏度(η)随剪切率的增大而降低;当剪切率一定,η随时间的延长而升高。剪切率(ω)一定,血液黏度分量(η’)、血液弹性分量(G’)随时间的延长而升高;时间一定,η’随ω的增大先降低后升高,在ω为1.6 2 7时增大到最高峰后再降低;G’随ω的增大先升高,在ω为3.0 0 0时,增大到最高峰后再降低。结论 在家兔肝撞击伤后的早期抗休克救治过程中,积极输血扩容的同时,还应采取措施降低血液黏度及黏弹性,以达到改善微循环,有效纠正组织缺血缺氧的不良状况,防止全身并发症的发生,Objective To investigate characteristics of blood viscoelasticity changes,the clinical significance and mechanism of affecting the body after hepatic impact injury in rabbits,and to provides an experimental foundation for clinical reducion of post-traumatic abnormal blood viscosity earlier.Methods The model of hepatic impact injury in rabbits was established with 2 steel balls falling down to the xiphoid process of rabbits.Blood(2ml)were taken respectively before injury,10,30,60,120min after injury and moment of death,and all of which were collected in heparinized tubes.All samples were measured within 24h.The finial results were calculated by the blood viscoelasticity calculation method.Results After hepatic impact injury in rabbits,blood viscosity decreased while shear rate's increased at the same time;and it increased with time at the same shear rate.The levels of blood viscosity(η')and elasticity(G') increased with time at the same shear rate;and at the same time,the level of η' increased with shear rates increased after short-time of decrease,and it reached its peaks when ω was 1.627;the level of G' increased as shear rates increased,and it reached its peaks when ω was 3.000.Conclusion Besides earlier anti-shock measures,it is one of the major measure for comprehensive treatment after hepatic impact injury to reduce post-traumatic abnormal blood viscosity,so as to improve hemorrheological status of the terminal circulation of tissues,protect and maintain organ function,and prevent the complications.
