摘 要:目的:探讨2型糖尿病患者胰岛素抵抗(IR)与血脂的关系。方法:回顾性分析2002年3月~2003年5月我院内分泌科2型糖尿病患者936例,根据体重指数分成肥胖组(459例)与非肥胖组(477例),比较两组的糖化血红蛋白(HbA1c)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白-胆固醇(HDL-C)、低密度脂蛋白-胆固醇(LDL-C)和胰岛素抵抗指数(HOMA-IR),分析HOMA-IR与血脂的相关性。结果:肥胖组的TG为(1.83±1.25)mmol/L显著高于非肥胖组(1.47±1.24)mmol/L,肥胖组的HDL为(1.40±0.36)mmol/L显著低于非肥胖组(1.49±0.39)mmol/L,两组TC、LDL差异无显著性。肥胖组的HOMA-IR为7.19±6.40,显著高于非肥胖组的4.88±4.07,HOMA-IR与TG呈正相关。结论:IR是引起2型糖尿病患者高TG血症的发病机制之一,肥胖加重IR,从而加重高TG血症。Objective To elucidate the relationship between insulin resistance(IR) and serum lipids in the patients with type 2 diabetes. Methods Nine hundred and thirty six cases of type 2 diabetes in our department from March 2002 to May 2003 were retrospectively analyzed,and the patients were divided into the obesity group (459 cases) and the nonobesity group (477 cases) according to body mass index. The serum glycohemoglobinA1C, total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C) and HOMA-IR were detected and compared between the two groups. The relationship between HOMA-IR and serum lipids was analyzed. Results The TG in the obesity group was (1.83±1.25) mmol/L, which was significantly higher than that in the nonobesity group (1.47±1.24) mmol/L. The HDL in the obesity group was (1.40±0.36) mmol/L, which was significantly lower than that in the nonobesity group (1.49±0.39) mmol/L. The TC and the LDL showed no significant difference in the two groups. The HOMA-IR in the obesity group (7.19±6.40) was significantly higher than that in the nonobesity group (4.88±4.07),which was positively correlated with the TG. Conclusions IR is one of the pathogenesis of the hypertriglyceridemia in patients with type 2 diabetes. Obesity may aggravate IR in the patients with type 2 diabetes, which would aggravate the hypertriglyceridemia .
