摘 要:目的观察清热解毒法拟定制成的脑宁康颗粒对大鼠脑出血后炎性反应因子:血浆细胞间黏附分子-1(ICAM-1)、肿瘤坏死因子(TNF-α)的影响,以探讨清热解毒法治疗脑出血的作用机制。方法实验大鼠随机分为假手术3d、7d组,模型3d、7d组,清热解毒3d、7d组,采用胶原酶Ⅶ诱导大鼠脑出血模型,术后2h,清热解毒3d、7d组给予脑宁康颗粒(2.4mg/kg)灌胃,另4组给予等量生理盐水,连续用药(1次/日),分别在3、7d后取血,应用酶联免疫吸附法和放射免疫分析法测定血浆ICAM-1、TNF-α含量。结果与假手术组比较,模型组血浆ICAM-1和TNF-α含量均明显升高(P<0.01),与模型组比较,清热解毒组血浆ICAM-1和TNF-α含量均明显下降(P<0.01,P<0.05)。结论清热解毒法对脑出血后神经细胞损伤具有保护作用,其机制可能与减少ICAM-1和TNF-α的产生,抑制炎症反应有关。OBSECTIVE To observe the effect of Naoningkang Granule on the inflammatory injury of rats after cerebral hemorrhage so as to ascertain the mechanism of the method of clearing away heat and removing toxin in the treatment of cerebral hemorrhage. METHOD Experimental rats were randomly divided into pseudo - surgery groups 1 and 2, model groups 1 and 2 and heat - clearing and toxin - removing groups 1 and 2. Collagenase Ⅶ was used to induce cerebral hemorrhage in rats. After two hours, heat - clearing and toxin- removing groups 1 and 2 were treated with Naoningkang Granule (2.4 mg/kg) and the other four groups were given saline once a day. Blood was taken at the third and seventh days respectively, which was then examined with enzyme - linked immunoassay and radioimmunoassay for the content of plasma ICAM - 1 and TNF - α. RESULT The content of plasma ICAM -1 and TNF - α in the model groups rose markedly compared with the pseudo - surgery group ( P < 0.01); the content of plasma ICAM - 1 and TNF - α in the heat - clearing and toxin - removing groups dropped markedly compared with the model groups (P<0.01,P<0.05). CONCLUSION The method of clearing away heat and removing toxin has protecting effect on the neural cell injury after cerebral hemorrhage, the mechanism of which is probably related to the inhibition of ICAM - 1 and TNF - α production and inflammation.
