髓内钉与动力髋螺钉治疗股骨粗隆下骨折的疗效分析

摘　　要：目的比较股骨近端髓内钉(proximalfemoralnail,PFN)与动力髋螺钉(dynamichipscrew,DHS)治疗股骨粗隆下骨折的疗效。方法随机将股骨粗隆下骨折的患者分为两组,PFN组和DHS组,PFN组26例,DHS组61例。对两组患者的手术创伤、功能恢复和并发症等情况进行分析。结果PFN组:平均手术时间为71.6min,术中出血量为179.2ml,手术切口6.5cm,早期起坐锻炼时间3d;DHS组:平均手术时间为78min,出血量190.2ml,切口长13.7cm,早期起坐锻炼时间4.5d。骨折愈合时间:PFN组3.1个月,DHS组3.3个月。内固定松动(皮质骨螺钉):PFN组无,DHS组3例;股骨头颈切割:PFN组1例,DHS组3例。结论采用PFN与DHS治疗股骨粗隆下骨折相比,骨折愈合时间无明显差异,但PFN较DHS创伤小,术后并发症少。Objective To study the the effects of tow methods of proximal femoral nail (PFN) and dynamic hip screw (DHS) to treatment the subtrochanteric femoral fracture.Methods All cases were divided into tow groups: PFN group and DHS group. 26 cases are come under PFN group; 61 cases are come under DHS group. Analyzing the results of the operating trauma, functional circs and complications. Results PFN: average operating time was 71.6 min, average bleeding volume 179.2 ml, average incision length 6.5 cm, forepart time of siting up 3 days. DHS: average operating time was 78 min, average bleeding volume 190.2 ml, average incision length 13.7 cm, forepart time of siting up 4.5 days. The times with fracture heal up was 3.1 months in PFN group, 3.3 months in DHS respectively. Internal fixation loosening:3 case in DHS group; to cut head and neck: 1 cases in PFN group, 3 cases in DHS group.Conclusion PFN is little trauma, less complications to compare with DHS into the subtrochanteric femoral fracture.
