摘　　要：目的:探讨蜕膜中杀伤细胞免疫球蛋白样受体(KIR)谱表达对妊娠结局的影响。方法:将95例自然或人工流产者分为3组:组1包括41例正常早孕人流妇女;组2包括16例原因不明的习惯性流产妇女;组3包括38例原因不明的1次自然流产妇女。用PCR-SSP方法检测蜕膜中KIR谱的表达。结果:组2蜕膜中KIR2DL2基因表达比组1明显增加(P<0·05),与组3无明显差异,而KIR3DL1基因表达比组1及组3明显下降(P<0·01);组2蜕膜KIR2DS5基因表达比组3明显增加(P<0·05),但与组1无明显差异。组3蜕膜中KIR2DL3和KIR2DS5基因表达分别比组1明显下降(P均<0·05),而KIR2DL3基因与组2无明显差异(P>0·05)。所有自然流产患者(组2+组3)蜕膜中的KIR2DL3和KIR3DL1基因表达与正常早孕组(组1)比较均明显下降(P<0·05,P<0·01)。结论:子宫蜕膜的KIR谱表达异常在导致妊娠失败的病理生理中起重要作用。Objective:To investigate the relationship between the expression of KIR′ repertoire in decidua and the outcome of pregnancy.Methods:A total of 95 women enrolled in this study,they were divided into 3 groups.There were 41 women with selective termination of normal pregnancy in group 1,16 women with unexplained recurrent spontaneous abortion in group 2 and 38 women with unexplained sporadic spontaneous abortion in group 3.The expression of KIR′ repertoire in decidua were determined by PCR-SSP.Results:The expression of KIR2DL2 gene was significantly elevated in deciduas of group 2 than that of group 1(P<0.05),while there was not statistical difference between group 2 and group 3(P>0.05).The frequency of KIR3DL1 gene was significantly decreased in group 2 than that in group 1 or group 3 (P<0.01).There was not statistical difference in frequency of KIR2DS5 gene between group 2 and group 1(P>0.05),whereas the frequency of KIR2DS5 gene was significantly increased in group 2 than that in group 3 (P<0.05).The expression of KIR2DL3 and KIR2DS5 genes in decidua of group 3 was significantly decreased than that of group 1(P<0.05),and there was no statistical difference in expression of KIR2DL3 gene between group 2 and group 3(P>0.05).The frequencies of KIR2DL3 and KIR3DL1 genes in the decidua of spontaneous abortion (group 2+ group 3) were significantly decreased than that in group 1 (P<0.05).Conclusion:The repertoire of expression of KIR genotypes in decidua may play an important role in maintenance of successful pregnancy or pathogenesis of failure pregnancy.
