摘 要:目的探讨高压氧(HBO)对大鼠急性下肢缺血再灌注致肺损伤的作用及其机制。方法大鼠随机分为假手术组、下肢缺血再灌注组(再灌注组)和下肢缺血再灌注+高压氧处理组(高压氧组)。夹闭大鼠腹主动脉2h后撤夹恢复血流,建立急性下肢缺血再灌注大鼠模型。应用电镜和流式细胞术,分别观察大鼠肺组织超微结构以及血小板膜糖蛋白CD31、CD61和CD62p表达的变化。结果与再灌注组相比,高压氧组肺组织超微结构的病理改变有所改善。再灌注组大鼠血小板膜糖蛋白CD31(36.45±6.37)和CD62p(68.98±8.73)表达均高于假手术组(25.60±10.25,52.93±12.71),有显著性差异(P<0.05),而高压氧组CD31(26.67±6.07)和CD62p(53.24±14.24)表达均低于再灌注组,有显著性差异(P<0.05)。假手术组、再灌注组和高压氧组3组之间CD61表达无显著性差异。结论早期HBO治疗可显著减少急性下肢缺血再灌注大鼠血小板膜糖蛋白CD31和CD62p的表达,可能有助于减轻急性下肢缺血再灌注所致的继发性肺损伤。Objective To investigate the effects of hyperbaric oxygen therapy on rat lung injury caused by acute hind limb ischemia-reperfusion and its mechanism. Methods The rats were randomly divided into 3 groups, sham operation group,hind limb ischemia-reperfusion group(IR group) and hind limb ischemia-reperfusion plus hyperbaric oxygen therapy group(HBO group). Acute hind limb ischemia-reperfusion rats models were established by clamping abdomial aorta for 2h before releasing the clamp. The changes of lung tissue ultrastructure were observed by electron microscope.The changes of the expression of CD31,CD61 and CD62p on platelet membrane were observed by flow cytometry. Results Pathologic changes of lung tissue of HBO group was significantly improved compared with those of IR group. The expression of CD31(36.45±6.37) and CD62p(68.98±8.73) on platelet membrane of IR group was significantly higher than that of sham-operation group(25.60±10.25, 52.93±12.71)(P<0.05). The expression of CD31(26.67±6.07) and CD62p(53.24±14.24) of HBO group was significantly lower than that of IR group(P<0.05). There was no significant change in the expression of CD61 among IR group,HBO group and sham-operation group. Conclusions Early hyperbaric oxygen treatment can effectively inhibit the expression of CD31 and CD62p on platelet membrane in rats after acute hind limb ischemia-reperfusion, which may help to alleviate the secondary lung injury caused by acute hind limb ischemia-reperfusion.
