硬膜外腔注入吗啡复合液用于术后镇痛的临床观察

摘　　要：目的:评价硬膜外腔注入吗啡及吗啡复合液的术后镇痛效果和不良反应。方法:选择200例妇产科手术患者,随机分为Ⅰ、Ⅱ组,每组100例,分别于手术结束前5min从硬膜外腔注入吗啡及吗啡复合液,注药前后观察呼吸、循环情况。术后72h随访,记录有关镇痛情况及恶心、呕吐、皮肤瘙痒等不良反应。结果:两组均有显著的镇痛效果,但两组镇痛时间比较有显著的差异(P<0.01),不良反应发生率有显著性差异(P<0.01)。结论:硬膜外腔注入吗啡2mg+氟哌啶2.5mg+地塞米松5mg+0.25%布比卡因共10ml用于术后镇痛,镇痛时间长,副作用相对较少,对呼吸、循环影响较少,镇痛效果确切,安全,为目前较好的术后镇痛方法,值得推广。Objective:To evaluate the postoperation analgesia effect of extradural injection of morphine complex liquid and its adverse effects.Methods:200 patients who undergone obstetric and gynecologic operations were averagely divided into two groups (5 minutes before operation with extradural injection of morphine and its complex liquid, respectively). The respiratory and circulation before and after operation were observed. The analgesia, nausea, vomiting and pruritus were noted by follow-up for 72 hours.Results: There were significant effects on analgesia in two groups with significant difference in analgesia time (P<0.01).There was significant difference in incidence of side effect (P<0.01).Conclusion:Extradural injection of morphine 2 mg + droperidol 25 mg + dexamethasone 5 mg +0.25% of bupivacaine has long analgesia time, less adverse effect, and less influence on respiratory and circulation system, which is a safe method with definitive therapeutic effect and worthy to be generalized.
