摘  要:目的:探讨数字化X线摄影(DR)中的双能量减影(DES)技术对气胸,特别是少量气胸的诊断应用价值。方法:用DR对60例经CT及临床证实的气胸患者进行DES检查,并与患者的常规DR胸片作对比分析。结果:对于大、中等量的气胸,常规DR胸片和DES图像均能全部显示,但DES图像能更好地显示气胸线(被压缩的肺边缘)、肺大泡、肋骨骨折等情况。对于少量气胸,DES图像的显示率为100%,明显高于常规DR胸片的45.5%,差异有统计学意义(P<0.05)。结论:DES技术能有效地去除胸部骨组织影的重叠遮挡影响,获得单纯的软组织图像,可提高气胸特别是少量气胸的检出率。Objective: To study the diagnostic value of dual-energy subtraction (DES) technique in digital radiography (DR) on pneumothorax. Methods: DES technique and CT were performed on 60 patients with pneumothorax. Results: Moderate or massive pneumothorax was seen by both conventional DR films and DES images, but the pneumothorax line (the compressed lung edge), pneumatocele and rib fracture were clearly shown by DES. DES was found to be better than conventional DR in detecting small pneumothorax (100% vs 45.5%, P<0.05). Conclusion: DES technique gains a pure soft tissue image via eliminating the overlapped shadow of osseous tissue, which improves the detection rate of pneumothorax, especially for small pneumothorax.
