摘 要:目的:探讨凝固时间法测定血浆抗凝酶(AT)活性在慢性肾功能不全尿毒症期患者血液透析中的临床意义。方法:将AT定标血浆稀释成不同活性,分别与纯品纤维蛋白原及凝血酶应用液反应,用血凝仪器测定其凝固时间,得出健康人和慢性肾功能不全尿毒症期血液透析患者AT活性实验结果。结果:血液透析组患者血浆AT消耗增多,导致AT活性明显下降,与健康人对照组比较差异有统计学意义(P<0.01)。结论:AT活性降低可反映慢性肾功能不全尿毒症患者的抗凝能力减弱,血液透析时用肝素抗凝效果不佳,应及时利用AT浓缩制剂替代肝素抗凝来保证血液透析的顺利进行和治疗安全。Objective:To assess the clinic significance of clotting time method for the determination of antithrombin (AT) activity in patients with uremic dialysis due to chronic renal insufficiency. Method:A different activity of AT was obtained from dilution of calibrated plasma antithrombin, and was reacted with pure fibrinogen and plasma coagulase, respectively. The curvilinear correlation was obtained between AT and clotting time. Clotting time was measured by coaguloviscosimeter. Result:The plasma AT activity in uremic dialysis was found to be significant lower than that in healthy group because of their increased plasma AT consumption(P<0.01). Conclusion:The detection of AT activity can reflect the capacity of antithrombin in patients with urinaemia due to chronic renal insufficiency. AT concentrated agent is a good sustitute for heparin when heparin is out of order in patients with urinaemia due to chronic renal insufficiency.
