彩色组织多普勒显像对左室扩大者室壁舒缩运动中心的观察

摘　　要：目的运用彩色组织多普勒显像技术观察判断左室扩大者室壁舒缩运动中心的位置.方法 A组18例左心室内径5.6～7.0 cm,B组11例左心室内径>7.0 cm.运用彩色组织多普勒显像技术观察左室壁舒缩运动,取心尖五腔观和心尖左室长轴切面,在收缩早-中期和舒张中期室壁运动彩色显像反转处与二维图像扇尖做一连线,垂直该连线再作一直线与参考左室长轴相交,该交点认为是左室舒缩运动中心.结果 A组左室收缩运动中心在参考左室长轴(从主动脉瓣前瓣附着处至左室心尖连线)的64.6%±3.2%位点处,舒张运动中心在63.9%±3.8%位点处,与正常人比较仅有向左室中部移位的趋势.B组左室收缩运动中心在参考左室长轴55.5%±3.2%位点处,舒张期运动中心在53.8%±2.2%位点处,与正常人比较明显向左室长轴中点移位,与A组比较亦有统计学差异.结论彩色组织多普勒显像技术可无创性估测以往需通过有创方法才能观察到的左室壁运动中心位置,对病理状态下左室扩大者室壁舒缩运动中心的估测可帮助临床医师了解病情,估计预后,具有一定临床价值.Objective To determine the center of segmental wall motion in dilated left venticle(LV) by color Doppler tissue imaging.Methods A total of 29 cases with enlarged left ventricle were divided into two groups according to the left ventricular transverse diameter (5.6-7.0 cm in group A,>7.0 cm in group B).All these cases were assessed by color Doppler tissue imaging.The images in early- and mid-systole and mid-diastole were analysed frame-by-frame off-line.A line was drawn to pass through the site in middle lager of LV wall where color changed and the point of the scanning imaging.Another line that was perpendicular to the first line as above was drawm to intersect the line from the anterolateral aortic valve edge to the apex.The intersection was considered as the center of the left ventricular segmental wall motion.Results The center of the LV segmental wall motion was at a point 64.6±3.2% of the distance along a line from the anterolateral aortic valve edge to the apex during systole,and 63.9±3.8% during diastole in group A,55.5±3.2% during systole and 53.8±2.2% during diastole in group B,which showed significant deviation in group B compared with the normal and group A.Conclusions Color Doppler tissue imaging can determine the center of segmental wall motion in dilated left ventricle done by invasive method at the past,which provide some useful information for clinical.
