三种常用幽门螺杆菌检测方法的评价

摘　　要：目的:对14C尿素呼气试验(14C-UBT)、快速尿素酶试验(RUT)、血清学检测抗幽门螺杆菌抗体(IgG)试验(HpAb-IgG)3种临床常用幽门螺杆菌(HP)检测方法进行回顾性评价。方法:对73例经内镜确诊的活动性消化性溃疡病病例进行14C-UBT、RUT、HpAb-IgG3项检测,采用四格表卡方检验对3项检测结果的敏感性、特异性及准确性进行统计学比较。结果:HpAb-IgG的敏感性显著高于14C-UBT(χ2=12.07,P<0.01)和RUT(χ2=21.06,P<0.01);HpAb-IgG的特异性显著低于14C-UBT(χ2=9.14,P<0.01)和RUT(χ2=9.14,P<0.01);HpAb-IgG的准确性高于RUT(χ2=4.96,P<0.05)。结论:HpAb-IgG检测结果中可能有部分为假阳性结果,14C-UBT和RUT检测结果中可能存在部分假阴性结果。Objective To explore the accuracy of 14Curea breath test(14CUBT),rapid urease test(RUT) and serological test for helicobacter pylori antibody(HpAbIgG) in helicobacter pylori(HP) infectionMethods Seventythree active peptic ulcer patients were determined by 14CUBT,RUT and HpAbIgGThe sensitivity,the specificity and the accuracy of the three tests were analyzed with Chisquare testResults The sensitivity of HpAbIgG was statistically and significantly higher than that of 14CUBT(χ2=1207,P< 001) and RUT(χ2=2106,P< 001);the specificity of HpAbIgG was significantly lower than that of 14CUBT(χ2=914,P< 001)and RUT(χ2=914,P< 001);and the accuracy of HpAbIgG was statistically higher than that of RUT (χ2=496,P< 005)Conclusion It is suggested that there may be some falsepositive results of HpAbIgG and some falsenegative results of 14CUBT and RUT in helicobacter pylori infection
