膝关节半月板损伤MRI诊断与关节镜检查对照研究

摘　　要：目的探讨膝关节半月板不同程度损伤的MRI表现,提高MRI检查对半月板损伤诊断的准确率。方法对200例临床可疑半月板伤患者进行了MRI检查,临床对80例Ⅱ度以上的患者进行了关节镜手术,将MRI诊断结果与关节镜检查进行了对照研究,分别算出MRI诊断的敏感性,特异性和准确性。结果在200例400个半月板中,MRI报告异常的有224个。其中I度64个;II度54个;III度72个,其中IIIA型53个(纵行撕裂21个,横行撕裂32个),IIIB型19个。另外有34个不能用上述方法分类,表现为半月板结构碎裂。以关节镜结果为标准,MRI诊断半月板撕裂的敏感性为96.7%,特异性为92.8%。结论MRI检查为一种无创性的能准确诊断半月板损伤的成像方法,将逐渐取代诊断性关节镜检查。Aim To retrospectively evaluate the MRI findings of meniscal tear of the knee and to raise the accurate rate of the MRI findings of meniscal tear of the knee. Methods MRI check was carried out in 200 cases with suspected meniscal injuries joint mirror surgical operation was performed in 80 patients with Ⅱ degree, and MRI findings and joint mirror findings were comared sensitivity, particularity and accuracy of a MRI diagnosis were calculated respectively.Results In of 200 case ,there are 224 cases with abnormality.Among them 64 I degree;54 II degree,and among 72 of III degree,53 were III A type with 21 of vertical tear and 32 of horizontal fear,19 III B type.Moreover 34 can't be classified with the above-mentioned method with the symtom of meniscus break.With the joint mirror result as standard, the sensitivity of MRI examination is 96.7%, and particularity is 92.8%.Conclusion The MRI check is an accurate diagnosis of meniscal tear and will replace the joint mirror check gradually.
