口服米非司酮对中期妊娠利凡诺引产效果的影响

摘　　要：目的探讨口服米非司酮对中期妊娠利凡诺引产效果的影响. 方法选择16～27 w因各种原因需终止妊娠的孕妇186例,随机分为观察组100例,予米非司酮150 mg顿服,12 h以后予利凡诺100 mg羊膜腔内注射;对照组86例,单用利凡诺100 mg羊膜腔内注射,观察两组胎儿排出时间、腹痛程度、出血量及胎盘残留情况. 结果①从利凡诺羊膜腔内注射到胎儿排出观察组平均(36.23±6.89)h,对照组平均(47.67±7.45)h,差异有显著性(P<0.01);②观察组产时出血量平均(70.83±10.79)ml,胎盘残留率23.00%,对照组产时出血量平均(112.87±20.79)ml,胎盘残留58.14%,差异有显著性(P<0.05). 结论米非司酮应用于利凡诺引产可以有效地促宫颈成熟,缩短引产时间,减轻腹痛程度,减少出血量及胎盘残留的发生.Objective To study the Clinical effect of oral Misopristol's cervix-stimulating on the induction of labor by rivanol in midpregnancy. Methods 186 gravida with 16～27 weeks pregnancy asked for induction of labor with all kinds of causes were randomly assigned into two groups. Gravida in Group A ( n= 100) took oral Misopristol 150 mg, and did amniocentesis induction of labor by rivanol 100 mg 12 hours later. Gravida in Group B (the control group)( n =86) only did amniocentesis induction of labor by rivanol 100 mg. Observed the time of fetus output, the extent of abdominal pain, the amount of bleeding and placenta remnants circumstances. Results ① The time from the amniocentesis induction of labor to the output of fetus in group A averaged (36.23±6.89)h, compared with that of a control group, (47.67±7.45)h, there was significant difference between two groups ( P <0.01). ② In group A mean intrapartum amount of bleeding was (70.83±10.79)ml, placenta remnants rate was 23.00%. In group B (the control group) mean intrapartum amount of bleeding was (112.87±20.79)ml, placenta remnants. Conclusion Misopristol's cervix-stimulating on the induction of labor by rivanol can promote the cervix mature, decrease the time of induction, reduce the extent of abdominal pain, decrease the volume of bleeding and the rate of placenta remnants.
