摘 要:2004年11月应用经食管裂孔食管切除术(transhiatalesophagectomy,THE)为1例贲门失驰缓症终末期患者行巨食管切除。全麻下于上腹部切口经食管裂孔钝性游离中下段食道,经颈部切口游离上段食道,切除巨食管后,将胃和颈部食道于颈部行改良式大口吻合,手术历时4.5h,出血620ml,随访6月,患者除发生短暂声嘶外其他情况良好。THE可用于巨食管的切除,具有不开胸、创伤小、恢复快的优点。1 case of terminam-staged cardia achalasia underwent resection of megaesophagus via transhiatal approach in November 2004.Upper abdominal incision was made under general anesthesia, middle and distal part of esophagus was bluntly mobilized via hiatus, proximal part of esophagus was mobilized through a neck incision. A modified anastomosis was made between stomach and cervical esophagus, operative duration was 4.5 h, blood loss was 620 ml, the patient recovered well except transient hoarseness. Transhiatal esophagectomy for resection of megaesophagus avoid thoracotomy, demonstrating less invasion and rapid recovery.
