巴氏水肿胎儿序列征流行趋势及干预对策

摘　　要：目的了解围产儿巴氏水肿胎儿序列征发生的变化趋势、分布特点和产前诊断水平,为进一步干预提供依据.方法利用广东省出生缺陷监测网的数据,有严格的三级质控,全部数据用SPSS统计软件分析处理.结果广东省1998～2003年巴氏水肿胎儿序列征的发生率总体变化不大,有波动.农村的发生率高于城市;山区、东西两翼地区高于珠江三角地区;女性患儿的比例高于男性患儿;产前确诊的比例为70.4%,目前主要依靠产前B超诊断,占69.0%;产前生化筛查的比例仅占2.1%;治疗性引产的比例为38.3%.结论巴氏水肿胎儿序列征的发生率与产前筛查诊断水平有关.对于巴氏水肿胎儿序列征的产前筛查诊断水平还有待进一步提高建议:1.加强宣教,实施人群监控和筛查,为高危夫妇提供咨询、产前检查、产前诊断等高质量的服务;2.加强对基层医务人员规范咨询、筛查的培训,提高产前筛查诊断的水平.3.建立地中海贫血咨询筛查诊断网络,实现三级医院规范转诊、信息技术资源共享,有效预防重症α-地中海贫血患儿的出生.Objective: To analyze the epidemic trend of Hb Bart hydrops fetalis sequence in Guangdong province, and set up a plan to reduce the incident rate of Hb Bart hydrops fetalis sequence. Methods: Analyzing monitor data of birth defect about Hb Bart hydrops fetalis sequence in Guangdong province. Results: The datum showed the incident rate of Hb Bart hydrops fetalis sequence are no obviously variational from 1998 to 2003. The rate in countries and mountain areas was higher than that in cities, the rate in developing areas was higher than that in developed areas. The rate of feminine fetus was higher than that of masculine fetus. The proporion of prenatal diagnosis was 70.4%. Mainly depended on B-ultrasound to final diagnosis. Conclusion: The incident rate of Hb Bart hydrops fetalis sequence related with the prenatal screening and diagnosis ability.The level of diagnosis Hb Bart hydrops fetalis sequence need improving In order to reduce the incidence rate, we must strengthen the prenatal health care propagandizing and genetic counseling, and find out a simple, timely, operative, accurate prenatal screening and diagnosing method, and improve the standard of prenatal diagnose, especially that in the primitive health organization. Creat a net of Hb Bart hydrops fetalis sequence to share resource and information about prenatal screening and diagnosing.
