异丙酚配伍米索前列醇用于人流镇痛420例临床分析

摘　　要：目的评价异丙酚用于人流镇痛的安全性,观察阴道放置米索前列醇的宫颈松弛作用。方法将要求人流镇痛的早孕健康妇女随机分为两组。A组:术前4h阴道后穹隆放置米索前列醇200μg,然后异丙酚静脉麻醉下进行人工流产手术。B组:术前无处理,直接异丙酚静脉麻醉下实施人工流产手术。两组进行多项指标对比分析。结果两组手术的麻醉效果,出血量均无显著差异(P<0·05)。但A组宫颈松弛度明显好于B组,手术时间也较B组明显缩短,两组比较有显著性差异(P<0·05);两组均无人流综合征发生,但A组和B组分别有2例的3例呼吸抑制发生。结论异丙酚用于人流镇痛,起效快,效果好,但要警惕呼吸抑制的发生,提高其安全性。术前4h阴道后穹隆放置米索前列醇200μg有较好的宫颈松弛作用。Objective:To evaluate reliability by administering Propofol to relieve pain in induced abortion and observe the effects of Misoprostol on relaxing cervix . Methods :Early pregnant women requested relieving pain in induced abortion were divided into two groups at random .Group A:Misoprostol was set at the back arched roof of the vagina at the 4h before operation with a dosage of 200μg , followed with anesthesia for induced abortion by intravenous injection of Propofol .Group B:Induced abortion was carried out by administering Propofol for anesthesia with no preoperative medication. Data out of the two groups were compared and analysed.Results : There was no significant difference in the anaesthesia effect and bleeding volume two groups(P<0.05).But the tension degree of cervix in groups A was less than group B and had a shorten duration of oderation compared with group B .That Showed significant difference between two groups (P<0.05); There was no syndrome accompanied with two groups, but there were 2 cases of respiratory depression occurred in group A and 3 cases in group B. Conclusion : Propofol used to relieve pain in induced can be effective .And yet ,it seems to be alert to bring respviatory depression under control for safety .The study suggests that administering Misoprostol at the back arched roof of the vagina at the 4h before operation with a dosage of 200μg can reduce the tension degree of cervix .
