益气补肾活血法辅助治疗早期糖尿病肾病的临床观察

摘　　要：目的观察在西医治疗的基础上使用中医益气补肾活血法治疗早期糖尿病肾病(EDN)的疗效。方法将108例EDN患者随机分为对照组56例和治疗组52例,对照组为常规西医治疗,治疗组加用益气补肾活血汤治疗,疗程8周。比较两组总疗效、症状改善率、尿白蛋白排泄率(UAER)、尿β2微球蛋白(Uβ2-MG)、血肌酐(Scr)、血清总胆固醇(TC)、血清甘油三酯(TG)和血液流变学变化。结果治疗组总有效率为91.07%,明显高于对照组的59.61%(P<0.01);各种症状明显改善,改善率90%以上,明显高于对照组(P<0.05或<0.01);两组UAER均明显降低(对照组P<0.05,治疗组P<0.01),但治疗组比对照组更为明显(P<0.05);治疗组Uβ2-MG、TC、TG和血液流变学指标均明显降低(P<0.05或<0.01),但对照组上述指标降低不明显(P>0.05)。结论益气补肾活血法结合西医治疗EDN疗效显著,可降低UAER和Uβ2-MG、改善血脂和血液流变学。Objective:To observe the efficacy of integrated Yiqi Bushen Huoxue Therapy of traditional Chinese medicine and western medicine(WM) in treating patients with early diabetic nephropathy(EDN).Methods:108 cases with EDN were randomized into control group (n=52) which was treated with WM alone and treatment group (n=56)which was added with Yiqi Bushen Huoxue Therapy. The clinical effective rates,symptoms amelioration rates,and the changes of urinary albumin excretion rate(UAER),urinary β_2-microglobulin ( Uβ_2-MG),serum creatinine(Scr),total cholesterol(TC),triglyceride(TG) and hemodynamic parameters were compared between two groups.Results:The overall effective rate (91.07%) of treatment group was significantly higher than that(59.61%) of control group(P<0.01).After eight weeks of follow-up,symptoms in treatment group were ameliorated in more than 90% of patients,which were significantly higher than that in control group respectively(P<0.05,orP<0.01).UAER in both groups significantly decreased (P<0.05,or P<0.01), but the decrement in treatment group was significantly superior than that in control group (P<0.05). The levels of Uβ_2-MG,TC,TG,and the indices of hemodynamics significantly decreased (P<0.05,orP<0.01) in treatment group, while no significant change was found in control group(P>0.05).Conclusion:The therapy of integrated Yiqi Bushen Huoxue and WM is effective for patients with EDN.It decreases UAER and Uβ_2-MG,and improves hyperlipidemia and hemodynamic parameters.
