摘 要:目的探讨再生周围神经中神经生长因子(nervegrowthfactor,NGF)促血管生成作用及其促血管作用对周围神经再生的影响。方法用单通道的硅胶管桥接75只Wistar大鼠1cm的坐骨神经缺损,在桥接管内给药,并将它们随机分为4组:生理盐水组(A组)、几丁糖组(B组,医用几丁糖简称几丁糖),NGF+几丁糖组(C组),NGF+几丁糖+抗VEGF组,前3组每组20只,术后7、14、30d,用免疫组化的方法检测大鼠再生坐骨神经新生血管内皮细胞中CD34、FactorⅧ、VEGF的表达,并作半定量分析。术后14d,Holmes染色再生坐骨神经纤维。结果生理盐水组、几丁糖组再生坐骨神经中表达的CD34、FactorⅧ、VEGF比较没有显著性差异,NGF+几丁糖组上述三抗原的表达比生理盐水组或几丁糖组有明显增加(P<0·05),NGF+几丁糖+抗VEGF组再生坐骨神经血管中的CD34和VEGF的表达完全被抑制,FactorⅧ的表达与NGF+几丁糖组相比也显著减少(P<0·0146)。Holmes染色发现,术后14d再生坐骨神经纤维数量及排列规则程度:NGF+几丁糖组>几丁糖组>生理盐水组>NGF+几丁糖+抗VEGF组。结论NGF可能通过促进VEGF产生发挥促周围神经再生作用。Objective To evaluate not only the effects of nerve growth factor (NGF) upon angiogenesis in the regenerative sciatic nerves of rats but also the impact of angiogenesis on regeneration of perpheral nerve. Methods Seventy-five rats were assigned randomly to four groups: normal saline group (n=20), chitosan group (n=20), NGF-chitosan Group (n=20), NGF-chitosan-antiVEGF group (n=15). The 10-mm defect of the sciatic nerves from every rat was bridged with silica gel conduit in the left latter limb. CD34, Factor Ⅷ, VEGF in the blood vessel of the regenerative sciatic nerves were observed by immunohistochemical staining at 7, 14, 30 d after operation. The regenerative sciatic nerves were dyed by the Holmes method at 14 d. Results The expression of CD34, Factor Ⅷ, VEGF in the regenerative sciatic nerves had not distinct difference in normal saline group and chitosan group and the expression of CD34, Factor Ⅷ, VEGF in NGF-chitosan group were significantly more than normal saline group and chitosan group (P<0.05). The expression of CD34 and VEGF in NGF-chitosan-antiVEGF group were absolutely inhibited in the regenerative sciatic nerves of rats, and the expression of Factor Ⅷ was markedly less than NGF-chitosan group(P<0.01). The quantity and regular array of regenerative nerve fiber in four groups from high to low at 14 d after operation by Holmes dyeing: NGF-chitosan group, chitosan group, physiological saline group, NGF-chitosan-antiVEGF group. Conclusion NGF may promote regeneration of the rasurg percpheral nerves the regenerative peripheral nerves through increasing VEGF.
