脑出血患者血清S100b蛋白与神经功能损害的相关性研究

摘　　要：目的探讨脑出血患者血清S100b蛋白与神经功能损害的相关性。方法①病例分组:ICH组,37例;对照组,30例(健康体检者)。②测试方法:采用双抗体夹心ELISA法测定患者血清S100b蛋白含量;患者急性期头颅CT扫描,根据多田公式(V=S×L×Slice×π/6)计算血肿体积;急性期采用CSS量表评价患者神经功能,恢复期(发病后3个月)采用Barthel指数评定患者日常生活能力。结果ICH急性期血清S100b蛋白水平显著高于对照组(P<0·01);ICH急性期血清S100b蛋白含量与血肿体积、CSS评分均呈显著正相关(n=37,r=0·965,P<0·01;n=37,r=0·949,P<0·01),与Barthel指数呈显著负相关(n=37,r=-0·972,P<0·01)。结论测定ICH患者急性期血清S100b蛋白含量有助于评价血肿体积、神经功能缺损及预后。Objective To investigate the relationship between serum protein S100b in intracerebral hemorrhage (ICH) patients and brain damage. Methods A total of 37 hypertensive cerebral hemorrhage patients, 24 male and 13 female, were enrolled as intracerebral hemorrhage (ICH) group, aged from 45 to 85 years with an average of (69.2±8.9) years. The control group were matched with ICH group, including 30 healthy subjects, 19 male and 11 female, aged 52 to 70 (61.2±5.0) years. The concentration of serum protein S100 was detected using double antibody sandwich ELISA. Evaluation of blood volume was calculated with the fomula based on cranial CT data: V=S×L×Slice×π/6. The nerve function of the patients was evaluated by the CSS during acute phase and Barthel index score at 3 month after stroke. Results Serum protein S100 concentration was significantly elevated in patients with ICH (0.54±0.41 μg/L), as compared to controls (0.17±0.04 μg/L)(P<0.01). Serum of protein S100 concentration showed a positive correlation with CSS (r=0.949, P<0.01) and blood volume (r=0.965, P<0.001), and a negative correlation with Barthel index score (r= -0.972, P<0.01). Conclusion The detection of serum protein S100 concentration of ICH patients at the acute phase is conducive to predict blood volume, degree of nerve lesion and the long-term neurological outcome.
