摘　　要：目的探讨Gamma钉治疗股骨粗隆周围骨折的疗效。方法1999年6月至2004年6月,Gamma钉内固定术治疗股骨粗隆周围骨折33例,男23例,女10例,平均年龄65.2岁。按Evan’s分类,I型5例,Ⅱ型7例,Ⅲ型14例,Ⅳ型6例,逆粗隆型1例。结果术中失血量约100～400ml,平均200ml,手术时间60～120min,平均90min。1例术中股骨粗隆下骨劈裂;1例切口浅表感染;2例髋内翻,颈干角丢失10°-15°,均为Ⅲ、Ⅳ型。33例经5～18个月随访。按韩一生等的方法对患髋进行综合评分,总优良率接近90%,效果满意。结论Gamma钉治疗股骨粗隆周围骨折创伤小,操作简便,固定稳妥,并发症少,疗效好,尤其适用于老年人股骨粗隆周围骨折。Objective Discussing about the results of treatment of inter-trochanteric fracture with Gamma-Nail system. Methods Altogether 33 cases of inter-trochanteric fracfure, 23make cases, 10 female cases,with average age 65.2-year-old.were treated with Gamma-Nail System from June, 1999 to June, 2004. According Evan's classification:Type I 5 cases;Type II 7 cases, Type III 14 cases,Type Ⅳ 6 cases, other 1 case; Results Blood volume lose vered frohaft fracture inlcase; superfacial sofe tissue infection in lcase; All 2 case with deformity of inner rotation 2 casesm 100ml to 400ml during operation, with average about 90min;femoral s neck-shaft angle olse 10° to 5°, all were typelII, IV. All 33 cases were folloned up from 5 to 18 months. Evaluation about the hip joint postoperation with Han yi-sheng's method. Excellent and good outcome was 90%. Conclusions The treatment of inter-trochanteric fracture with Gamma Nail system, is eviderced by clecreasing osft tissne trauma;stable inner fixation, few complication,fine outcome,easy were minapulation,estecialey for elder people.
