彩色多普勒超声诊断动脉导管未闭的临床价值

摘　　要：彩色多普勒超声诊断３４例动脉导管未闭，并有手术证实。超声诊断正确性９７％，漏诊１例（３％）。超声与手术所见未闭导管大小的比较，小于１５ｍｍ的导管，超声与手术测值相关良好；１５ｍｍ以上者，超声轻度低估手术测值。彩色多普勒直观显像未闭导管五彩镶嵌分流束入肺动脉是其诊断最可靠征象，弥补了二维超声难以区别管道还是韧带的不足，提高了诊断准确性。Color Doppler flow imaging(CDFI)is used to diagnose 34 Patients of Patent ductus arteriosus(PDA) and there are operations to dimonstrate the result.The accuracy of supersound diagnosis is 97%, only one is ignored(3%). Comparing the results of the supersound diagnosis with those of the operations about the dimension of the PDA,they fit well when the ductus are smaller than 15mm, in those larger than 15mm, supersound underestimated. The fast that CDFI Can display Visualizedly the Current of PDA entering Pulmonalis arteria is the most reliable sign of this diagnosis,which Compensates the deficiency that two-dimensional echocardiography diagnosis Can't distinguish ductus from ligament; besides, it raises the accuracy of diagnosis quickly, reliably and without injury.So, the author thinks that CDFI is by now the best method to diagnose PDA.
