摘  要:目的研究早期经口进食对急性胰腺炎病情的影响。方法24只雌性SD大鼠,随机分成两组,通过电针刺激制作胰腺炎模型。第1组行早期经口进食,第2组行肠外营养,48h后处死大鼠,取血行生化及酶学检测,取胰腺和左肺行病理学检查,比较两组结果。结果血LDH、SGOT和淀粉酶水平第1组明显高于第2组,血糖和血钙水平第1组明显低于第2组;胰腺的炎症反应第1组轻于第2组,但肺的炎症改变与之相反。结论经口进食所致的血清酶的升高对胰腺有一定的保护作用,但酶的升高对内皮的损伤又加重了胰外脏器的损害。Objective This study was undertaken in an attempt to determine the impact of early oral feeding on the severity of acute pancreatitis. Methods 24 female Sprague-Dawley rats were divided into two groups. In both groups, acute pancreatitis was induced by using electric needle with 3 voltage and 20 Hz to stimulate common bile pancreatic duct through duodenum of rat. The rats in group Ⅰ were fed orally and the rats in group Ⅱ were fed parenterally. The rats were sacrificed at 48 hours, and blood samples were obtained for the measurement of biochemical parameters. The pancreas and the left lung were removed for histopathological examination. Results The levels of LDH、SGOT and amylase were significantly higher whereas glucose and calcium levels were significantly lower for the rats in group Ⅰwhen compared with the rats in group Ⅱ(P<0.05). Inflammatory changes observed in the pancreas were less severe whereas inflammatory changes observed in the lung were more severe for the rats in group Ⅰ when compared with the rats in group Ⅱ. Conclusion The study suggests that certain hormones released from the duodenum upon stimulation by oral feeding lessens the severity of pancreatitis through protective effects on the pancreas whereas the elevated levels of the enzymes cause endothelial damage resulting in destruction.
