颈部防护对高值加压呼吸的代偿效应

摘  要:8名被试者穿着侧管背心-抗G服-加压面罩,分别进行了不同余压值(7.8kPa,9.8kPa)和不同余压增长率(0.3kPa/s,5.0kPa/s)的高值加压呼吸(PPB)实验,对心率、平均动脉压、每搏指数、心输指数、总外周阻力和TⅡ幅度进行记录分析。结果表明,在颈部防护状态下进行PPB,各分析指标与颈部未防护状态相比均有不同程度改善,余压值越高,余压增长率越快,颈部代偿效应就越好。其主要原因与颈部防护有效地限制PPB时气管的扩张,相对平衡了颈动脉内外压差,减轻了对颈动脉窦的刺激,降低了迷走神经兴奋性有关。subjects wearing capstan jerkin,G-suit and oxygen pressure mask underwent PPB at vari-ous excess pressures(7.8kPa and 9.8kPa )and PPB at 9.8kPa with various excess pressure in- creasing rates(EPIR 0.3kPa/s and 5.0kPa/s)under the conditions with or without neck protec-tion.The heart rate(HR), stroke index(SI),cardiac index(CI), mean arterial pressure(MAP)andTⅡ voltage(Tv)were analysed. The results demonstrated that most recorded parameters wereimproved during PPB with neck protection.The higher the excess pressure during PPB and thequicker the excess pressure increasing rate of PPB,the more obvious the compensatory effects ofneck protection.The compensatory effects of neck protection during high level PPB were mainlyrelated to the attenuation of the vagal excitation by balancing the transmural pressure of carotidartery to alleviate the carotid sinus stimulation.
