手助式微创经皮肾穿刺取石术治疗上尿路结石

摘　　要：目的探讨手助式微创经皮肾穿刺取石术(MPCNL)治疗上尿路结石的疗效.方法回顾性分析64例采用手助式MPC-NL治疗的上尿路结石患者的临床资料.其中男40例,女24例;单侧54例,双侧10例;肾多发结石34例(包括孤立肾结石1例),肾多发结石并输尿管上段结石15例,输尿管上段结石15例.结石直径1.0～2.5 cm.结果64例均一期手术成功,手术时间30～90 min,平均60 min.术中估计出血量平均20 mL,无大出血、气胸及腹腔脏器损伤等并发症发生.住院时间5～9 d,平均7 d.术后3 d复查X线腹部平片或B超,结石清除率92.2%(59/64);术后1个月复查,结石清除率100%(64/64),无严重并发症发生.结论手助式MPCNL治疗上尿路结石安全有效,创伤小,恢复快.Objective To evaluate hand assistant mini-percutaneous nephrolithotomy in treating upper urinary tract calculi. Methods Data of 64 patients who underwent hand assistant mini-percutaneous nephrolithotomy for upper urinary tract calculi were reviewed. Results All the 64 patients were cured by single stage operation of hand assistant mini-percutaneous nephrolithotomy. The average operation time was 60 minutes, ranging from 30 to 90 minutes. The estimated volume of blood loss was 20mL and the average hospital stay was 7 days ranging from 5 to 9 days. Post-operation clearance rate of calculi in 3 days and one month was 2.2% and 100% respectively. No major complications were noted. Conclusion Hand assistant mini-percutaneous nephrolithotomy is safe and effective in treating upper urinary tract calculi.
