摘　　要：目的探讨2型糖尿病合并脂肪肝与血清瘦素的关系。方法65例2型糖尿病,其中合并脂肪肝者30例,除外嗜酒者、病毒性及药物性肝炎史者,用酶联免疫方法(ELISA)测定人血清瘦素,同时测定空腹血糖(FPG)、空腹胰岛素(FINS)、游离脂肪酸(FFA)、总胆固醇(TC)、甘油三酯(TG),计算HOMA模型胰岛素抵抗指数(HOMA-R)。对照组健康人30例,与糖尿病组相匹配,分组和观察指标也相同。结果2型糖尿病脂肪肝患者的瘦素、FINS、HOMA-R、TG显著高于无脂肪肝组(P<0.05)。以2型糖尿病是否伴有脂肪肝为因变量,进行多元线性回归分析显示,BMI、瘦素、TG是糖尿病患者脂肪肝发生的影响因素。结论2型糖尿病脂肪肝患病与肥胖、胰岛素抵抗和瘦素水平有关。Objective:The aim of the study was to investigate correlation between type 2 diabetic with fatty liver and leptin level.Methods:Sixty five type 2 diabetic patients,35 with fatty liver.The leptin concentration are measured by ELISA.In conjunction with serum leptin level,BMI,and plasma glucose,and serum insulin and lipids,(triglycerides,total cholesterol) free fatty acid (FFA) were measured.sixty control group.Results:Type 2 diabetic with fatty liver higher than without fatty liver in leptin,FINS,HOMA-R,and TG.Logistic regression analyses showed diabetes mellitus with fatty liver were positively correlated to BMI,serum leptin level,TG.Conclusion:These results indicate that serum leptin levels and insulin resistance and lipid disorder are associated with type 2 diabetic with fatty liver patients.
