温热病防传杜变的临床探讨及机理研究——附172例临床分析报告

摘　　要：本研究选择流行性出血热(EHF)为研究对象,系统研究表明:温热病传变的病理基础为疫毒邪火和气阴耗伤,出血、瘀血系传变的病理标志;以清热解毒、清气凉营、益气养阴和活血化瘀等多法联用,对172例患者采用清瘟散等制剂,多途径投药,分阶段进行应急处理和治疗,治疗组总有效率为94.17％,病死宰为0.97％(p<0.01)。并通过病理学的深部体温、甲皱微循环、细胞酶组织化学以及动物病理模型的分子生物学和病毒学等方面的研究,阐明温热病传变的实质是:严重感染,引起脏器功能和实质受损、衰竭,微循环障碍,继发弥散性血管内凝血,进而损伤多种重要细胞器,并有氧自由基损害的参与。同时还揭示了:所选中药新制剂具有的抗病毒,降解内毒素,改善微循环,保护受损器官、组织、细胞器,调节机体的免疫功能等作用,是能有效地防传杜变的机理所在。This study selected 172 cases of epidemic hemorrhagic fever(EHF) as study subjects. Results revealed that the pathological bases of the progress of epidemic febrile disease are pathogenic fire and impairment of qi and yin,hemorrhage and blood stasis are the pathological expression. Varied therapeutic methods (clearing heat and toxic material,clearing qi and cooling yingfen, supplementing qi and nourishing yin, promoting blood circulation and removing blood stasis,etc, )were combined to treat the patients. The herbal prescriptions were administered in different ways to the patients. The tatal effective rate was 94. 17% and the mortality was only 0. 97% (p<0. 01). Studies on deep temperature,nail fold micro-circulation.endoenzyme histochemistry, molecular biology and virology of animal pathological model demonstrated that severe infection causes the damage and failure of the organ function and parenchyma,the disturbance of microcirculation and secondary DIC, and further causes the damage of varied organelles and oxygen free radical. It also revealed that the Chinese herbal prescriptions have the functions of resisting virus,degradat-ing endotoxin,improving microcirculation,precting the damaged organs, tissues and organelles,and regulating the immune function of antibody.
