鼻内窥镜下电凝止血治疗重症鼻出血48例体会

摘　　要：目的:探讨鼻内窥镜下电凝止血治疗重症鼻出血的方法及效果。方法:对48例重症鼻出血患者使用鼻内窥镜检查寻找出血部位,使用自制的套有一次性输液器的金属吸引器管,用前端金属头接触出血点,边吸血边用电刀触碰吸引器后端金属部分,以达电凝止血效果。结果:48例中,鼻内窥镜下电凝止血一次成功46例,回病房后再次出血,又行二次止血成功2例。结论:采用鼻内窥镜下电凝止血,能明确各鼻道及鼻顶等隐蔽部位的出血,有效止血;减少患者因鼻腔填塞紧、时间长引起的局部及全身并发症。Objective:To explore the method and effect of electrocoagulation hemostasis in treating severe epistaxis under nasal endoscope. Method: Under nasal endoscope, the bleeding points of 48 patients were found. A metal suction tube coated by disposable perfusion tube was made by ourselves, its anterior part was used to suct and contact the bleeding points, its posterior part contacting electrotome was used for to electrocoagulation hemostasis.Result:Among 48 cases, 46 had hemostasis once successfully, 2 bled again and were cured by twice treatment. Conclusion: Electrocoagulation for hemostasis under nasal endoscope can confirm the masked bleeding points in meatus or roof of nose, and coagulate them effectively. The local and general complications induced by long tensive packing are released.
