摘  要:目的探讨麻醉技术应用于消化内镜检查的可行性。方法随机选择接受胃镜、结肠镜检查者1378例,其中自愿接受静脉麻醉的683例为麻醉组,选用手背静脉推注丙泊酚2mg/kg;接受普通胃镜、结肠镜检查的695例为对照组。记录患者接受检查前后不同时点的血压、心率、血氧饱和度,观察操作时间、清醒时间、不良反应及患者的主观意见反应。结果麻醉组不良反应发生率明显降低(P<0.01),检查中血压、心率较检查前均有不同程度下降,患者的满意度和愿意再接受检查的比例明显高于对照组(P<0.01)。结论丙泊酚静脉麻醉应用于消化内镜检查安全可行,可明显减轻痛苦和不良反应,有利于提高内镜检查的成功率。Objective To evaluate the safety and feasibility of painless technique with propofol anesthesisa in gastrointestinal endoscopy.Methods 1 378 patients undergoing gastroscopy or colonoscopy were randomly divided into two groups:anesthesia group and control group. In the anesthesia group, propofol (2 mg/kg) was intravenously injected before gastroscopy or colooscopy. In control group, patients underwent conventional endoscopy. Blood pressure (BP), heart rate (HR) and oxygen saturation (SaO_2) were monitored before and after endoscopy. Duration of examination, time for recovery of consciousness, adverse reactions and patients' self-feeling were recorded.Results The occurrence of adverse reactions decreased significantly (P<0.01), the proportion of satisfaction and re-acceptancet increased significantly (P<0.01) in anesthesia group, compared with control group. In anesthesia group, BP and HR decreased to some extent during endoscopic examination.Conclusion The painless technique with propofol anesthesia in gastrointestinal endoscopy is safe and feasible. It markedly reduces patents' discomfort and adverse reactions,and raises success rate of endoscopy.
