畸变产物耳声发射筛选未通过新生儿的听觉诱发电位

摘　　要：目的探讨脑干听觉诱发电位(brainstem audio evoked potential,BAEP)在婴儿耳聋早期诊断的应用价值、方法500例婴儿从出生到6wk 内耳声发射(otoacoustic emissions,OAE)3次未通过者,拟诊耳聋的患儿进行听阈及 BAEP 检查。如听阈在65dB SPL 以上或 BAEP 异常者,1～2个月后来复查。如患儿月龄在6-12个月以上,听阈仍在65 dB SPL 以上者就进行干预及治疗。结果 1000耳中正常98耳、轻度耳聋515耳、中度耳聋234耳、重度耳聋80耳、极聋72耳。1～2个月后来复查的有754耳,正常的占233耳、轻度耳聋271耳、中度耳聋203耳、重度耳聋23耳、极度耳聋24耳。与首次比较差异有显著性(P<0.01)。结论 BAEP 检查可以检测婴幼儿听力损害的程度,及跟踪随访,并早期干预,早期治疗及早期语言训练成为可能,使患儿聋而不哑。Purpose To explore the value of brain audio evoke potential(BAEP)in earlier diagnosis for hearing loss infants.Methods 500 eases who failing three times in test of distortion product of otoacoustie emissions(DPOAE)from birth to 6 months,or being diagnosed to deaf were accepted to the test of BAEP.Re-test was taken for patients with hearing thresh.hi more than 65 dB SPL or abnormal BAEP after 1-2 months.Patients with more than 6-12 months old or hearing threshold more than 65 dB SPL were received intervention and treatment.Results 98 ears were normal,515 ears were mild hearing loss,234 ears were moderate heating loss,80 ears were severe hearing loss and 72 ears were profound hearing loss among 1 000 ears.There were 754 ears that received re-test after 1-2 months,which shows that 233 normal ears,271 mild hearing loss,203 moderate hearing loss,23 severe hearing loss and 24 profound hearing loss.There were significant defference between two groups of first test and re-test(P<0.01).Conclusion BAEP can test the degree of hearing loss impairing for infants and follow the trace of diseases to interfere earlier so that making it possible to earlier treatment and training in speech.
