摘 要:目的:细胞间黏附分子1作为免疫球蛋白超家族之一,在炎症早期介导白细胞与受损内皮细胞的紧密黏附。分析细胞间黏附分子1基因K469E多态性分布情况及吸烟与中国汉族人群心肌梗死患病率的关系。方法:选择2002-10/2004-05济南中心医院心内科选择住院心肌梗死患者69例为患者组,男40例,女29例。选择同期本院健康体检自愿者77人为对照组,男46人,女31人。两组对象均为汉族且无血缘关系。利用巢式聚合酶链技术对两组对象进行细胞间黏附分子1基因K469E多态性检测和对比分析。结果:按意向处理分析,进入结果分析心肌梗死患者69例,健康人77人。①基因型频率符合Hardy-Weinberg平衡,患者组与对照组的KK+KE和EE基因型频率分别为0.812,0.188;0.649,0.351,差异明显(χ2=4.816,P=0.028,OR=2.326,95%CI:1.084~4.992)。②调整其他因素后,Logistic回归示K等位基因和吸烟均不同程度的增加了心肌梗死的患病危险性,同时吸烟与K等位基因之间对于心肌梗死的患病存在着一定的协同作用,而且高于吸烟和K等位基因单一作用(K等位基因:P=0.041,OR=2.408,95%CI:1.035~5.602;吸烟:P=0.012,OR=3.028,95%CI:1.270~7.732;K等位基因和吸烟协同作用:P=0.002,OR=3.339,95%CI:1.536~7.258)。结论:中国汉族人群中存在细胞间黏附分子1基因K469E多态性,基因型和等位基因在心肌梗死患者及正常人中的分布不同,K等位基因可能为心肌梗死的独立危险因素之一,而且它与吸烟在影响心肌梗死发生危险性方面有协同作用。AIM: As a member of the immunoglobulin super family, intercellular adhesion molecule-1 mediates the tight adhesion between leucocyte and damaged endothelial cells at early period of inflammation. In the study,we analyzed the distribution of intercellular adhesion molecule-1 gene 469K/E polymorphism, and investigates the association between smoking and the prevalence rate of myocardial infarction in Chinese Han population. METHODS: Sixty-nine patients with myocardial infarction (40 males and 29 females), who were hospitalized in the Department of Cardiology, Jinan Central Hospital between October 2002 and May 2004, took part in this study, and 77 voluntary physical examinees (46 males and 31 females) in this hospital at the same period were selected as controls (control group). All the subjects in both groups were Han people and had no blood relationship. The intercellular adhesion molecule-1 gene 469K/E polymorphism was detected with nested polymerase chain reaction and comparatively analyzed. RESULTS: According to intention-to-treat analysis, all the 69 patients with myocardial infarction and 77 healthy subjects were involved in the analysis of results. ① The genotypic frequency met the Hardy-Weinberg balance, and the genotypic frequencies of KK+KE and EE were obviously different between the patient group and control group (0.812, 0.188; 0.649, 0.351; χ2=4.816, P=0.028, OR=2.326,95% confidential interval =1.084 to 4.992). ②After other factors were adjusted, Logistic regression showed that both K allele and smoking increased the prevalence risk of myocardial infarction; Meanwhile smoking and K allele had certain synergistic action for the prevalence of myocardial infarction, which was higher than the single action of smoking or K allele (K allele: P=0.041, OR=2.408, 95% confidential interval= 1.035 to 5.602; Smoking: P=0.012, OR=3.028, 95% confidential interval= 1.270 to 7.732; Synergistic action of K allele and smoking: P=0.002, OR=3.339, 95% confidential interval=1.536 to 7.258).
