摘 要:目的:研究热水灌胃导致大鼠萎缩性胃炎(chronicatrophicgastritis,CAG)胃黏膜组织细胞超微结构的变化,探讨CAG发病机制及长期过热饮食与萎缩性胃炎发生的关系.方法:热水灌胃(55℃蒸馏水,2.5mL/d)建立大鼠萎缩性胃炎动物模型,选取鼠腺胃胃窦组织进行病理组织学检查及扫描电镜和透射电镜观察胃黏膜组织细胞超微结构的变化.结果:热水组于灌胃后24wk,光镜见大鼠胃黏膜出现萎缩表现.扫描电镜见胃黏膜细胞萎缩、腺腔增大并见出血现象;32wk时细胞萎缩加重,大量上皮细胞脱落、细胞破损、局灶性糜烂.透射电镜见腺体和腺细胞萎缩,胞质内细胞器减少,分泌颗粒减少并空泡化,粗面内质网明显扩张,并出现早期凋亡现象.32wk时胞质内细胞器锐减,早期凋亡现象较前普遍.结论:长期过热饮食可造成胃黏膜的损伤,并有超微结构的损害,可导致胃黏膜萎缩.AIM: To observe the ultrastructural changes of gastric mucosa in atrophic gastritis model caused by hot water and to investigate the pathogenesis of atrophic gastritis and the relationship between chronic atrophic gastritis (CAG) and hot diet. METHODS: The atrophic gastritis rat model was made by hot water(55℃, 150g/mL, 2.5mL/d)perfusion and the ultrastructure of gastric mucosa was observed by scanning electron microscope and transmission electron microscope. RESULTS: ① When hot water was given for 24 weeks, the gastric mucosa shrunk markedly. ② The scanning electron microscopic observation revealed that the gastric mucosa cells became atrophy, the adeno-vessels broadened and bleeding could be seen when hot water perfusion was given for 24 weeks. The mucosa atrophy worsened and the fall of epithelium, the breakage of the cells and the erosion of mucosa also could be seen when hot water perfusion was given for 32 weeks. ③ The transmission electron microscopic observation revealed that the gastric gland shrunk, glandular cells were atrophy, organells and secretory granules decreased, secretory granules had vacuolation and rough endoplasmic reticulum dilated when water perfusion was given for 24 weeks. Early apoptosis was also observed. When the hot water was given for 32 weeks, organells reduced clearly and more early apoptosis was observed. CONCLUSION: Long time hot diet may injure gastric mucosa, cause damage to cell ultrastructure in chronic gastric mucosa and thus lead to atrophy of grastric mucosa.
