放置宫内节育器后出血与病毒感染的关系

摘　　要：采用病毒分离培养方法对１１３例放置宫内节育器（ＩＵＤ）后出血病例（ＩＵＤ出血组），放置ＩＵＤ无副作用的１０９例（ＩＵＤ正常组）、未放置ＩＵＤ正常妇女２４例（无ＩＵＤ正常组）及未放置ＩＵＤ异常子宫出血１１例（无ＩＵＤ异常出血组）的子宫内膜进行病毒分离。各组标本用兔肾细胞进行病毒分离，病毒阳性率依次为５９．２９％、１９．２７％、１６．６７％、６３．６４％。分离出的病毒用多聚酶链反应（ＰＣＲ）技术鉴定巨细胞病毒（ＨＣＭＶ），各组阳性例数依次为２６例（３８．８１％）、４例（１９．０５％）、１例（１／４）、３例（３／７）；用中和试验方法鉴定单纯疱疹病毒Ⅱ型（ＨＳＶ－Ⅱ），阳性例数依次为１６例（２３．８８％）、５例（２３．８１％）、１例（１／４）、０例；尚未鉴定明确者各组依次为２５（３７．３１％）、１２（５７．１４％）、２（２／４）、４（４／７）。ＩＵＤ出血组病毒（ＨＣＭＶ、ＨＳＶ－Ⅱ）检出率，显著高于ＩＵＤ正常与无ＩＵＤ正常组，而与无ＩＵＤ异常出血组相近。bjective To study the relationship between uterine bleeding with intrauterine device(IUD)and virus infection. Methods Virus isolat ion from endometrium samples both with IUD and without IUD was per- formed with rabbit kidney cell.The cell pathogen effect(CPE)directed the positive isolation. All isolat-ed viruses were funher identified by polymerase chain reaction(PCR)for human cytomegalovirus(HCMV)and by neutralizing test for herpes simplex virus type Ⅱ(HSV-Ⅱ)。Results The endometrial samples were abtained from four groups:113 patients with uterine bleed- ing with IUD;109 healthy women,without uterine bleeding with IUD 124 healthy women without IUD and 11 patients with uterine bleeding without IUD.The rates of virus isolation in four groups men- tioned above were 59.29%,19.27%,16.67%and 63. 64%respectively. The positive rates for HCMV and HSV-II in thefourgroups were 38.81%,19.05%,1/4,3/7 and 23.88%,23.87%,1/4,0.00% respectively.The unidentified viruses were 37.31%,57.14%,2/4 and 4/7 respectively. Conclusions These results showed that some end ometrial samples from both the bleeding groups and the controls were infected by HCMV andH SV-II. The incidence of virus infection in women with uterine bleeding with IUD was much higher than that of the controls,but similar to that is the patients with uterine bleeding without IUD. The results suggested that IUD could activate latent viruses(HCMV,HSC-II)infection and induce bleeding in certain conditions.
