摘  要:总结104例骨纤维结构不良的临床及病理表现,并对6例新鲜标本作碱性磷酸酶染色,3例新鲜标本作透射电镜观察。结果发现104例中36例骨小梁周围有成骨细胞镶边,且纤维样细胞与典型成骨细胞移行;电镜下纤维样细胞周围有大量胶原纤维丝,其中可看到电子密度不均匀,圆形或不规则形态结构,这些结构即是从成骨细胞而来的基质小泡。6例新鲜标本碱性磷酸酶染色均呈阳性。16例中见粘合线。根据(1)纤维样细胞周围的基质小泡的形成同成骨细胞周围的基质小泡完全一致。(2)纤维样细胞以及成骨细胞均含有丰富的碱性磷酸酶。因此认为两种细胞的性质相同。推测在骨的纤维结构不良中的纤维样细胞其本质符合骨祖细胞特点。故认为骨纤维结构不良的发病机制可能是成骨过程的障碍。即骨祖细胞向成骨细胞转化受阻。AbstractThe clinical manifestations and the pathologialfindings in 104 cases of fibrous dysplasia were re-viewed.6 fresh specimens were stained with alkalinephosphatase and 3 were studied with transmissionelectron microscope(TEM).The results revealedthat,among the 104 cases,36 had osteoblasts pavedalong the bone trabeculae and tranistion of fibroblastsinto typical osteoblasts,Under TEM,large amount ofcollagen fibers can be seen around the fibroid cells;circular or irregular particies of different electronicdensities were abundant.Evidently,these structureswere originate from the matrix vesicles of the os-teoblasts. Positive alkallne phosphatasestain was ob-served in the 6 fresh specimens,Cementing fibrilswere seen in 16 specimens, Based on the similarities ofthe distribution of matrix vesicles around the fibroidcells and the osteoblasts,and the abundance of alka-linephosphatase in both cells, it is reasonable to con-sider an identical origin of these two cells. The au-thors concluded that,fibrous dyspalsia might be re-sulted from a disturbance of osteogenesis or an inter-ruption of the transition of the bone stem cells into theosteoblasts.
