骨筋膜室综合征中缺血－再灌注损伤的实验研究

摘　　要：作者采用改进的Ｈａｒｇｅｎｓ模型对筋膜切开减压后是否诱发缺血组织再灌注损伤进行了研究。结果显示单纯行筋膜切开减压组，其血浆中总ＳＯＤ活力下降，ＭＤＡ升高，肌纤维损伤较重，毛细血管损伤明显，提示有氧自由基诱导的膜脂质过氧化反应的发生，导致了缺血－再灌注损伤。因此在临床工作中既要恢复缺血组织的再通血，又要注意防止缺血－再灌注损伤。AbstractHargen’s model was used for the study of the oc-currence of ischemia-reperfusion injury following fas-ciotomy for the treatment of compartmental syndrome.The results revealed that, in the group underwent sim-ple fasciotomy, there was decrease of SOD activity andincrease of MDA,severe myofibril disorganization andmarked capillary injuries. These were caused by lipidperoxidation induced by free oxygen radicals leading toischemic-reperfusion injury. Eight cases of compart-mental syndrome with compartmental pressure between4. 79 and 11. 97 pa were treated with mannitol and vita-min E following fasciotomy, giving satisfactory re-sults. Restoration of reperfusion to the ischemic tissueand prevention of ischemic-reperfusion injury are equal-ly important in chical practice.
