带血管蒂大转子骨瓣转移对股骨头不同病变的治疗

摘　　要：在３２侧经红色乳胶灌注过的成人下肢标本上，解剖测量了旋股外侧血管横支的起始，走行位置和分支，观测了供应股骨大转子前外侧血供的来源，可切取大转子骨瓣３．５ｃｍ×２．０ｃｍ×４．０ｃｍ。并设计了带该血管蒂骨瓣转移治疗不同病变的股骨头缺血性坏死。从１９９１年１月以来，临床应用２７例经１年５个月至３年６个月的随访，取得了满意的疗效。此法的作用是增加股骨头的血运，还能重建股骨头和修复软骨的作用。AbstractThe authors studied the course and distribution ofthe transverse branch of lateral femoral circumflexartery in 32 adult cadaveric lower limb specimens。Theblood vessels supplying the anterolateral part of thegreater trochanter covered a total volume of 3.5cm ×2.0cm ×4.0cm。 The authors designed the technique oftransposition of vascular pedicled trochanter flap forthe treatment of various ischemic necrosis of thefemoral head.Since January l991,27 cases were sotreated.They were followed up for l year 5 months to3vears 6 months。The clinical results were satisfacto-ry. The advantage of this method is the augmentationof the blood supply of the femoral head,which is valu-able for the osteogenesis and chondrogenesis。
