摘  要:在15例新鲜颈椎标本上定量地研究了颈椎后部结构切除对其前屈、后伸和左、右侧弯运动的影响,包括与单开门术式相关的椎板及部分黄韧带、与颈椎神经根减压术有关的单侧或双侧小关节内侧半等结构。研究表明两侧椎板和部分黄韧带切除对颈椎运动无显著性影响,即颈椎单开门术不会造成颈椎的不稳;单侧小关节内侧半切除后颈椎运动无明显改变,但双侧小关节内侧半切除则会产生节段的失稳,其前屈、后伸和侧弯运动将分别增加33%、74%和44%。说明小关节在颈椎活动中起重要作用。建议在双侧小关节内侧半切除后应植骨融合或器械内固定,以加强颈椎稳定性。AbstractThe effects of the posterior vertebral strctures on the movements of the cervical spine in flexion/ex-tension and right/left lateral bending was studied with fresh cervical spine specimens of 15 young cadavers。The posterior structures considered were those closely related with the expansive open-door laminoplasty and the unilateral or bilateral hemiresection of interfacet for decompression of the cervical nerve roots,The re-sults of this study indicated that:1.the range of mo-tion of the cervical spine was not changed significantly following the resection of bilateral laminae and related yellow ligaments or unilateral hemiresection of inter-facet;2.the motion of cervical spine increased signifi-cantly following bilateral hemiresection of interfacets,being 33%,74%and 44%in flexion,extension and lateral bending respectively.Therefore,in such cir-cumstances,an interspinal process fusion or an instru-mentation must be used to strengthen the stability of the cervieal spine.
