改良型Hoffmann外固定器治疗不稳定性胫腓骨骨折

摘　　要：自１９９２年３月以来，作者采用国产改良型Ｈｏｆｆｍａｎｎ外固定器治疗不稳定性胫腓骨骨折５４例，其中开放性骨折３４例。随访时间平均１７个月，骨折完全愈合平均２５周，膝和踝关节功能良好，钉道感染１４．８％。认为这是一种简便、有效的治疗方法。在比较了三种不同构型外固定器治疗方法后，推荐使用双面半针型，其明显优于其它构型。钉道感染和骨延迟愈合仍是外固定器治疗的两大并发症，对此可通过正视钉周围坏死组织的处理和减少钉一界面松动来降低钉道感染的发生率，可采用正确的复位和给予骨折断端间必要的压应力以避免延迟愈合发生。AbstractSnce Mar.1992.54 cases of unstable fractures ofthe tibia and fibula,inchding 34 cases of open fractures,were treatd by a modified Hoffmann external skeletalfi xation. All patients were followed up with an averageperid of 17 months. Complete healing of fractures wasseen in an average of 25 weeks;the joint function of an-kle and knee was satisfactory and the pin tract infectionwas 14.8%。The modified Hoffmann extermal skeletalfixation was considered a convenient and effectivemethod. Having compared the results among three typesof external skeletal fixation, the authors believed that two separate unilateral systems was better than the oth-ers and was worthy of recommending. Pin tract infectionand delayed union continued to be two major complica-tions of external fixation svstems,but the former wasminimized by special attention to management of necrosistissue around the pin and by decrease of the pin-bonemotion,the latter was avoided by accurate reductiOn andby necessary com pression effect bet ween the fractu redfragments.
