大剂量皮质激素吸入治疗激素依赖型哮喘的临床观察

摘　　要：大剂量皮质激素吸入加特非那丁、酮替酚等药物治疗２３例激素依赖型哮喘，试图撤减口服激素，结果表明：完全撤减成功的占５６％，部分撤减的占２６％，临床症状改善的占１３％，完全无效的占４．４％；治疗后肺功能较治疗前显著改善（Ｐ＜０．０５）；治疗前平均血浆皮质醇基值为０．１４７±０．０４３μｍｏｌ／Ｌ，治疗后升至０．２０３±０．０５１μｍｏｌ／Ｌ（Ｐ＜０．０１）；ｓｙｎａｃｔｈｅｎ试验０、６０和１２０分钟时相上唾液皮质醇值比治疗前显著上升（Ｐ＜０．０１）。结果提示大剂量皮质激素吸入是治疗激素依赖型哮喘的较好方法之一。n order to provide a more effec tive alterna tivetherapy for the steroid dependent asthmatic(SDA)pa-tients,23 SDA patients replaced their oral steroids withhigh dose beclomethasone dipropionate(15 00 μg/d)in-halation were investigated. The changes of clinical fea-tures,pulmonary function and the Synacthen test wererecorded,The results showed that about 82% of the pa-tients replaced their oral steroids completelyor partiallywith the inhalation therapy,the clinical features wereimproved in 13%of the patients and the failure rate wasonly 4.4%,the results also revealed that,after replace-ment,the puimonary function of t he patients were im-proved(P< 0.05);according to the results of Synac-then test,after alternative therapy for 3～6 months,the damaged reserve power and secretive ability of a-drenal cortex of the patients were also partially improved(P<0.01).
