摘 要:目的:测定正常人脑组织不同部位代谢物浓度、计算其比值。材料和方法:用多体素磁共振质子波谱PRESS序列测定100例正常脑组织的额叶、颞叶、顶叶、枕叶、基底节区和小脑代谢物浓度,观察NAA、Cho、Cr的波峰特点,计算和分析NAACr、ChoCr、NAACho的比值。结果:正常脑实质的1H波谱Levene方差分析显示额叶、颞叶、顶叶、枕叶、基底节区和小脑的NAACr(P<0.000)和NAACho(P=0.001)有显著差异,ChoCr在上述各部位的浓度无显著差异(P=0.068)。进一步用Bonferroni方差分析比较各组间显著差异性显示小脑NAACr低于额叶、顶叶、枕叶的NAACr(P值分别<0.05,<0.01,<0.05),小脑NAACho低于顶叶、枕叶的NAACho(P均<0.01),而额叶、颞叶、顶叶、枕叶、基底节区之间NAACr、NAACho无显著差异。结论:多体素1HMRS可以测定NAACr、ChoCr、NAACho浓度比值,不同解剖部位代谢物浓度不尽相同,为颅脑代谢异常提供参考标准。Purpose: To determine the regional normal brain metabolite concentrations and measure the metabolic concentration ratios. Materials and Methods: One hundred cases of normal brain were examined by multi-voxel proton MR spectroscopy with PRESS sequence. 1 H MRS imaging acquisitions were performed in six brain locations: frontal lobe, temporal lobe, parietal lobe, basal ganglion, occipital lobe and cerebellum. 1 H MR spectra were processed to determine the metabolite ratios, including NAA/Cr, Cho/Cr and NAA/Cho. Results: The ratios of NAA/Cr and NAA/Cho showed significant difference in frontal lobe, temporal lobe, parietal lobe, basal ganglion, occipital lobe and cerebellum (P<0.000,0.001 respectively) by Levene analysis. The ratios of NAA/Cr in cerebellum were lower significantly than that in frontal, parietal and occipital lobe (P<0.05,<0.01,<0.05,respectively)and the ratios of NAA/Cho in cerebellum were lower significantly than that in parietal and occipital lobe (both P< 0.05) by Bonferroni analysis. Cho/Cr was not statistical difference among frontal lobe, temporal lobe, parietal lobe, basal ganglion, occipital lobe and cerebellum. No significant difference of NAA/Cr and NAA/Cho was found in frontal lobe, temporal lobe, parietal lobe, basal ganglion and occipital lobe. Conclusion: Multi-voxel MR spectroscopy could measure NAA/Cr, Cho/Cr and NAA/Cho. The metabolic concentration ratios in different normal brain regions could be as a internal standard.
