摘 要:应用血液透析滤过(HDF)对31例尿毒症患者进行了500次治疗,与常规血液透析(HD)比较,HDF治疗中低血压的发生率低。HDF组治疗后血β_2微球蛋白、中分子物质明显下降,而HD组无此作用。HDF对BUN、Cr的清除率明显高于HD,其Kt/V值与HD组比较差异不显著,说明HDF3小时可取得HD4~4.5小时的清除效果,以尿素动力学模型评价可达到充分透析。This study was undertaken to compare the clinic effect of hemodiafiltration (HDF) with that of hemodialysis (HD). 31 patients on chronic hemodialysis had been switched to HDF. A significant decrease of the frenquency of hypotension was observed during HDF. The meanplasma β_2microglobulin (β_2MG ) and middle molecular substance (MMS) levels decreased during HDF (P<0.01) . On the contrary , no significant decreases of plasma β_2MG and MMS were found during HD. HDF offered higher clearance for creatinine and urea than HD (P<0. 01). No significant difference of Kt/V values was found between HD and HDF groups. In conclusion,HDF present an advanced technique that offers the highest clearance of metabolic wastes and good vascular stability. HDF treatment for 3 hours can give adequate dialysis for high Kt/V values as well as HD treatment for 4 hours.
