摘 要:介绍采用改良式颈椎椎体次全切除术治疗47例多节段脊髓型或脊髓-神经根混合型颈椎病。手术有效率为93.7%(44例),优良率为80.9%(38例),无手术死亡或重残发生。文中讨论了本手术的解剖基础、适应证和作者手术体会。Abstract Subtotal vertebrectomy was performed in 47 patients with multisegmental cervical spondylotic myelopathy or myeloradiculipathy.There were 37 men and 10 women,ranging in age from 35 to 76 years.There were 30 cases with cervical spondylotic myelopathy and 17 cases with cervical spondylotic myeloradiculopathy.In this series,1-3 vertebral bodies were subtotally removed with modify its posterior one third removed in woodwind-like fashion high-speed microdrill under sursical microscope.The spinal cord and nerve roots were completly decompressed,a rectangular autogenous bone graft was inserted in the bone defect.Follow-up ranged from 4 to 48 months, with an average of 18 months.In the total 47 patients,44(93.7%)patients showed improvement and 38(80.9%)showed remarkable improvement.No malunion of bone grafts.The authours suggest,the modifyed subtotal vertebrectomy may be an idearl surgical procedure for treatment of multipe segmental cervial spondylotic myelopathy or myeloradiculopathy.
