经颈内静脉肝内门体分流术适应症与并发症的防治

摘　　要：作者采用经颈内静脉肝内门体分流术（ＴＩＰＳ）对１４０例晚期肝硬变门静脉高压症患者进行治疗。ＴＩＰＳ成功率是９４．６２％，并发症的发生率为１６．１５％，与ＴＩＰＳ操作有关的并发症发生率为４．６２％，主要包括腹腔内出血、胸腔内出血、食管静脉曲张再出血；与门体分流有关并发症的发生率为１１．５４％，主要包括轻度肝性脑病和急性肝功能衰竭，术后一个月内死亡率是３．０８％，死于急性肝功能衰竭和食管静脉曲张再出血分别为２．３１％和０．７７％。作者重点讨论了ＴＩＰＳ的适应症、并发症的发生和防治。AbstractTransjugular intrahepatic portosystemic shunt(TIPS)was carried out in 140 cases of advanced cirrho-sis with portal hypertension. The success rate of TIPSwas 94. 62%。 The complication rate of TIPS was16.15%,of which,4.62%was TIPS procedure relatedincluding intraabdominal or intratheoracic bleeding andrebleeding of esophageal varices. The complication raterelated to portosystemic shunt was 11. 54%and thecomplications included encephalopathy and acute hepat-ie failure, The mortality of TIPS was3. 08% within onemonth following TIPS.2.31%of patients died of hepat-ic failure and 0. 77%died of rebleeding of varices.
